(This isn’t intended to be bureaucratic - it just helps ensure that it doesn’t get lost. Info that is sent via the form goes into a special mailbox; stuff that comes in my regular e-mail or that is posted on the board can easily get lost in the shuffle.) I feel badly when people don’t get their certificates or don’t get them on time because I don’t have the correct information.
